A trap has two ways to fail, and they look nothing like each other.

A trap that fails open passes live steam into the return. A trap that fails shut holds condensate back in the exchanger. The first shows up as a bill and a hot return line. The second shows up as an exchanger that has stopped making temperature.

What it does
Live steam passes straight through to the condensate return. The heat in it was generated and paid for, and none of it did any work.
How it shows
The return line runs hotter than it should and stays hot. On a shared return the effect turns up at other plant, not at the trap that caused it.
Why traps end up open
Dirt on the seat holds the valve off, or the float mechanism has worn. Both are the reason a strainer sits ahead of the trap.

What it does
Condensate cannot leave. It backs up into the exchanger and floods the surface that was supposed to be transferring heat.
How it shows
Output falls off and the exchanger stops making temperature. The complaint arrives as a process fault rather than a trap fault.
The other consequence
A flooded line and a slug of condensate in a steam space is how waterhammer starts. That is a mechanical risk, not only a thermal one.

What the check needs
Temperature either side
The simplest check is the temperature upstream and downstream of the trap. It needs the body reachable, not buried.
Listening to it
An ultrasonic check tells a passing trap from a working one. It also needs contact with the body.
Why the cover comes off
A trap under fixed lagging is a trap nobody tests. The module here is cut to the body and opens by hand, so the check stays a five-minute job.
